To receive your mail in rebate, purchase above listed NeilMed^® products at your local CVS Pharmacy store, submit original UPC Bar Code along with printed copy of this page and a copy of your receipt with your name, complete address, email and phone number to the below mentioned address. Please allow up to 30 days to receive the rebate check. Rebates do not include tax. Products must be purchased between September 26, 2011 to November 15, 2011 and submitted by November 30, 2011.
