Send the original UPC barcode, a printed copy of this page, and a copy of your receipt, along with your name, complete mailing address, email address, and phone number to the address below to receive your $3 manufacturer's mail-in cash rebate. Rebate request must be postmarked within 45 days of the purchase date.
Please allow up to 30 days to receive your rebate check.
